• The prescriber’s information: Name, contact address, phone number, and signature.
• Your name and address. The name on the invoice must match the name on the prescription.
• A general description of the item and the quantity. It must be for the item you are purchasing tax-free.

We review all prescriptions before shipping your purchase out. If the prescription has any issues, we will contact you to clarify the issue.

Manufacturer Warranty

MedX Warranty Guarantees their Rehab Laser System for 3 Years.

All MedX Laser and Light Products include a three-year limited warranty, with batteries and accessory cables covered for twelve (12) months.
MedX will complete all in-warranty repairs or replacements within thirty (30) days of receiving the defective product.

Please check your package immediately to be sure your order is correct and items were not damaged. If you receive a defective product or an incomplete order, CONTACT US IMMEDIATELY prompt action. Damaged or short shipments should be reported within 5 days of receipt. Please keep the box if you are contacting us about a damaged item.

Please send photos of the defect on the product label to [email protected]. We will communicate with you if the item(s) is/are eligible for repair/ replacement. If approved, we will provide you with a return shipping to send back the defective product, you will receive a replacement after the defective product is received.

Please carefully review your order before confirming your purchase. All sales of this product are considered final.

Exceptions to the No Refund Policy

The only exceptions to our No Refund Policy include:

• Defective or Damaged Items: In the case of items that are received defective or damaged, please report this within [Insert Time Period, e.g., 5 days] of receiving the item.
• Wrong Item Sent: If you receive an item different from the one you ordered, please notify our Customer Service within 5 days for a possible refund or exchange.
• Expired Products: Receiving a product in an expired condition (unless stated and part of a clearance price) warrants a report to our Customer Service within 5 days for potential refund or replacement.
• Legal Requirements: Following applicable laws, any other conditions or circumstances where we are legally required to offer a refund will be honoured.
