• The prescriber’s information: Name, contact address, phone number, and signature.
• Your name and address. The name on the invoice must match the name on the prescription.
• A general description of the item and the quantity. It must be for the item you are purchasing tax-free.

We review all prescriptions before shipping your purchase out. If the prescription has any issues, we will contact you to clarify the issue.

Manufacturer Warranty

OrthoActive Warranty Guarantees their Bracing Products for 3 months.

OrthoActive offers a limited 3 month warranty on the products it manufactures. Products manufactured by OrthoActive are warranted to the original purchaser against defects in workmanship and material for 3 months after receiving the product. The foregoing warranty shall not apply to normal wear and tear, or if the product has been subjected to misuse or misapplication the warranty will be voided at the discretion of OrthoActive.

Please check your package immediately to be sure your order is correct and items were not damaged. If you receive a defective product or an incomplete order, CONTACT US IMMEDIATELY prompt action. Damaged or short shipments should be reported within 5 days of receipt. Please keep the box if you are contacting us about a damaged item.

Please send your order number, photos of the defect on the product label to [email protected]. We will communicate with you if the item(s) is/are eligible for a warranty replacement. If approved, we will provide you with a return shipping to send back the defective product, you will receive a replacement after the defective product is received.

Return Policy

30 Day Return Policy from date of arrival!

Returned items must be in excellent and/or unused condition for full product credit. Use extra caution when packing for returns to avoid damage. Law labels and identification labels must remain on all products for credit. Credit cannot be issued for products with markings, stickers or torn pages. If the original packaging or instructions are missing or damaged, or if the items are not in ‘like new’ condition, packed in a secondary box, a restocking fee of 25% will be deducted from your refund.

All orders shipped outside of Canada are final sale and cannot be returned or exchanged.
